General Instructions:

If you need a sacramental record from the Cathedral of Christ the King, please fill out this form and we will processs your request. The user of the certificate may want it sent directly to them with our seal affixed. Please ask them how they want it sent to avoid a delay in the delivery.

We plan to send out the sacramental records no later than 2 weeks after the request.

If you want to pick up the record at the office, it is open Mondays through Fridays from 9am to 5pm, Saturdays from 9am to Noon.
